Community Nursing Service

What is the Service?

This community nursing service provides:

• Nursing follow-up and care after discharge from hospital
• Ongoing nursing care for people with a chronic illness or disability
• Supervision of medication
• Wound management
• Advice and education about health related problems
• Nursing management and symptom control for people with cancer and terminal illness
• Support for families and friends
• Assessment for Carer Allowance
• Consultation and discussion about options and choices of services for clients and carers
• Liaison and referral to other community services and doctors.

Who can receive this service?

• All residents of the Lane Cove Municipality
• Pensioners and non-pensioners
• Workers' Compensation patients
• Insurance patients.

When is it available?

Service is available throughout the year.
8am to 4:30pm Monday-Friday
Reduced services on Saturday, Sunday and public holidays.

How does it operate?

The service employs experienced, qualified registered and enrolled nurses. After receiving a referral, a Registered Nurse will complete a comprehensive nursing assessment and develop a Care Plan for each patient.
A schedule of visits and care is then based on the assessment and regularly monitored by the nursing staff.
The service will at all times endeavour to meet the individual requirements of each patient, but some flexibility may also be needed due to the unpredictable nature of the service.

Do I have to pay to receive this service?

There are minimal fees to receive this service, depending on the type of service provided and the eligibility of the patient. However, any concerns regarding inability to pay may be discussed with the Nurse Unit Manager.
Wound care products and dressings and other products used as part of the service will be charged at cost price.
